Letter of Intent

I ensure / agree on the following:

  1. That I shall take primary responsibility for the education of my child/children under the Homeschooling Program;
  2. That I shall manage the education of my child/children following the rules and regulations prescribed by the Department of Education and compliant to the K to 12 Basic Education Program and the policy of zero tolerance for any form of child abuse, exploitation, discrimination, violence, bullying and other forms of abuse under DepEd. Order No. 40, S. 2012 or the DepEd Child Protection Policy in managing the learning of my child/ward.
  3. That I shall perform, all the roles and responsibilities of a parent/guardian of a homeschooler as contained in Annex 3 of this DepEd Order.
Homeschooling Agreement

In this agreement, the party who is contracting to avail of the Homeschooling Program services of the school shall be referred to as "Parent", and the Party that will be implementing the Homeschooling Program shall be referred to as the "Homeschooling Program Implementing School."

DECLARATION OF DUTIES OF PARENT/GUARDIAN

  1. I declare that I am the parent/guardian of the above-named homeschool learner;
  2. That I am enrolling my child/children with due consideration of the best interest of my child/children and their views on their education, taking into account their age and maturity level;
  3. That I shall take full responsibility for the education of my child/children under the Homeschooling Program and that I shall perform the roles and responsibilities of parents/guardians of Homeschool Learners as enumerated below:
    1. Submit relevant documents as required for enrollment to the Homeschool Program
    2. Ensure that my child/children shall undergo the K to 12 curriculum with enhancement if deemed necessary.
    3. Manage the education of my child/children.
    4. Prepare my child/children's school year schedule in the home setting.
    5. Express my willingness to follow the school calendar.
    6. Provide learning opportunities for my children and organize my children's home learning experiences and opportunities.
    7. Attend orientations/training for parents/guardians of homeschool learners organized by the school.
    8. Ensure the availability of adequate and appropriate learning resources for my children.
    9. Report the progress of my child/children to the school.
    10. Consult the homeschool coordinator, whenever needed.
    11. Ensure the administration of learning assessments.
    12. Plan, implement, and conduct the formative assessment of my children.
    13. Ensure the submission of requirements needed for my children's assessment.
    14. Submit the quarterly portfolio of my children to the school through the homeschool coordinator.
    15. Coordinate and/or consult with the homeschool coordinator as agreed upon and as needed.
    16. Adhere to the policy of zero tolerance for any form of child abuse, exploitation, discrimination, violence, bullying and other forms of abuse under the DepEd Order No. 40, s. 2012 or the DepEd Child Protection Policy in managing the learning of my child/children.
    17. Perform all other duties and responsibilities of a parent/guardian under the Homeschooling Program in accordance with DepEd Order 001 s. 2022.

DECLARATION AND DUTIES OF THE SCHOOL

  1. That the school, through the Homeschool Coordinator shall:
    1. Ensure the homeschool learner's inclusion in the Learner's Information System (LIS);
    2. Monitor the progress of the homeschool learners;
    3. Serve as teacher-coach to the parents/guardians;
    4. Administer the quarterly assessment to the homeschool learners on an agreed time and venue; and
    5. Conduct home visitation, when necessary.
  2. Perform all the duties and responsibilities of the parent/legal guardian under the Homeschooling Program, in accordance with DepEd Order 001 s. 2022.
  3. That I have read and understood the above roles and responsibilities and that I will ensure the performance of my roles to the best of my ability.
Parent / Guardian Declaration

I certify that the information provided in this application, including the learner's current and actual residence, is true and complete. I understand that I must promptly inform PPIS of any material change in the learner's residence and of anticipated international travel or relocation that may affect participation in the Homeschooling Program.

I understand that submission of this application, payment of fees, creation of a PPIS learner profile, participation in orientation, or submission of partial requirements does not by itself constitute formal enrollment in the PPIS Homeschooling Program.

I further understand that formal enrollment is subject to verification of eligibility and residency, completion of required documents and procedures, and the existence and scope of applicable DepEd authority.

Continuing Disclosure Undertaking

I understand that eligibility for the PPIS Homeschooling Program is subject to applicable PPIS and DepEd requirements. I undertake to provide truthful and current information regarding my child's actual residence and to promptly inform PPIS of any material change of residence, anticipated international travel or relocation, change of primary learning facilitator, or other circumstance that may materially affect my child's participation in the Homeschooling Program.

I understand that international travel notification is required for school monitoring and regulatory purposes and does not constitute an application to PPIS for permission to travel.

I further understand that PPIS may request updated residency or other supporting documents when reasonably necessary to verify continuing eligibility.

The standard PPIS Homeschool application requires the documents below, as applicable to the learner's grade level and enrollment type. Anything not uploaded here may be submitted to the Homeschool Coordinator during enrollment; the application is not final until the requirements are complete.

Admissions Policy Regarding Specialized Learning Support

Prince of Peace Integrated School Inc. is dedicated to providing a high-quality, accessible education to our student community. To ensure every child receives the care and instruction they deserve, please note that while we strive for inclusivity, the school currently lacks the specialized diagnostic tools and clinically trained personnel required to effectively support students with significant physical or developmental needs.

To ensure we do not overpromise on a learning environment we cannot effectively provide, admission for students requiring specialized accommodations is reviewed on a case-by-case basis and is not guaranteed. We kindly request that parents or guardians provide full disclosure of any known physical or developmental conditions during the enrollment period so we can determine if our facilities and staff can meet your child's specific needs.

Please be advised that failure to disclose a known condition during the enrollment period may lead to a cancellation of enrollment. In such instances where information was withheld, the school reserves the right to retain all paid fees in accordance with the 1992 Manual of Regulation for Private Schools (DepEd), regardless of whether the student is withdrawn or transferred before the second week of classes.
